Medical appointment reminder services cost $29 to $150 a month for a standalone plan that covers 400 to 3,200 text or voice reminders, which works out to about 5 to 7 cents a reminder, according to the prices vendors publish in October 2026. Pay-per-reminder services charge about 4 to 19 cents, and several practice management systems include reminders at no extra charge. For a three-provider clinic sending 2,400 reminders a month, the published options range from about $40 (raw Twilio, if you build it yourself) to $408 (a per-reminder rate card). The dedicated reminder tools that put a BAA in writing cost $150 to $208.
We checked the pricing pages of more than 40 reminder vendors on October 7, 2026, all on the vendors' own websites. That covers standalone reminder tools, texting and voice broadcast services, and the reminder modules inside practice management and EHR software. Fifteen standalone services and nine practice platforms publish a usable figure. The large patient engagement suites (Solutionreach, Weave's plan prices, Luma Health, NexHealth, RevenueWell) quote privately, and they are in the tables anyway, because knowing that before a demo saves you a call.
Which appointment reminder services publish a price?
The services that publish are mostly self-serve tools a practice manager can sign up for in an afternoon. Read the "billed for" column before the price, because a credit, a reminder, a message and a calendar are four different units.

Two patterns stand out. The dedicated reminder tools (Appointment Reminder, Apptoto) price by reminder volume and include voice calls in the same allotment, so a practice whose older patients still want a phone call does not pay extra for it. The bulk texting tools are cheaper per message but say plainly that they are not built for patient information, which matters more than the price difference once you read the BAA column.
Is your EHR's reminder module cheaper than a separate service?
Often, yes. If your practice management or EHR system already sends reminders, the marginal cost can be zero or a few cents a text. Check this first, because a standalone tool that syncs with your calendar is an extra integration to maintain.

The two that charge a visible per-reminder fee are TherapyNotes (14 cents) and TheraNest Advanced past 50 (10 cents). Both are higher than any dedicated reminder plan once you send more than a few hundred a month. At 2,400 reminders, TherapyNotes' add-on alone comes to $336, more than double Appointment Reminder's top plan.
How much does a voice reminder call cost?
Voice reminders cost 4 to 19 cents a call at published rates, and most services count a short call the same as a text. Appointment Reminder counts a call as one reminder from the monthly allotment. Apptoto counts a call up to 2 minutes as one credit ($0.12 past the plan). DialMyCalls, CallMultiplier and Text-Em-All charge one credit per 30 seconds of recorded message, 3.8 to 9 cents depending on volume, and CallMultiplier and Text-Em-All bill only when the call is answered or reaches voicemail. STI's ChartMaker module charges 15 to 19 cents per successful reminder. At the raw end, Twilio charges $0.014 a minute plus $0.0075 for answering machine detection, and TextMagic $0.041 a minute plus 4 cents for text to speech.
Keep the recorded message under 30 seconds. On every credit-based voice service, a 35-second message costs exactly twice as much as a 28-second one.
What 2,400 reminders a month costs on each plan
The worked example is a three-provider clinic seeing about 1,200 patients a month and sending two reminders per visit (one a few days before, one the day before), each short enough for a single 160-character text. That is 2,400 reminders a month.

Three things fall out of this table. First, among dedicated tools that put a BAA in writing, the spread is narrow: $150 to $208 a month. Second, the cheapest texting plans undercut them by $30 to $40 a month and give up the BAA to do it. Third, if your EHR already includes reminders, a separate service only makes sense for something the module cannot do, such as voice calls for older patients or a waitlist that fills cancelled slots.
Do appointment reminder services need a BAA?
Yes, if the service handles patient information for you, and a reminder carrying a patient's name, appointment time and your practice name usually does. Under HIPAA, a vendor that creates, receives or transmits protected health information on a covered entity's behalf is a business associate, and you need a signed business associate agreement with it before you send.
The vendors split cleanly. Appointment Reminder, OhMD, Spruce, IntakeQ, Carepatron and Twilio state a BAA as part of the plan or the account. Apptoto signs one only from 16 calendars, which is why its BAA price starts at $208. Text Request, Jane and Practice Better offer one on request. SimpleTexting and EZ Texting say in their own help and industry pages that SMS through their service is not built for medical information, so keep anything beyond "you have an appointment tomorrow at 9" off those platforms, or pick another vendor.
How much does a no-show cost a practice?
The most cited peer-reviewed figure comes from the Michael E. DeBakey VA Medical Center in Houston, which measured an average no-show rate of 18.8% across ten clinics and an average cost of $196 per missed appointment in 2008 (Kheirkhah and colleagues, BMC Health Services Research, 2016). Your own figure depends on your visit fee and how fast you can refill a slot, but the order of magnitude is useful. At $196 a visit, a $150 reminder plan pays for itself if it prevents one missed appointment a month.
That is also why the cheapest plan is rarely the right decision rule. A $35 monthly difference between two vendors is a fraction of one recovered visit. Two-way confirmation (the patient replies C or R), a voice option for patients who do not text, and a reliable sync with your schedule move the no-show rate. The last few dollars of price do not.
Do you pay for 10DLC registration?
Every business texting from an ordinary 10-digit number in the US has to register a brand and a campaign, and someone pays for it. Through Twilio the pass-through fees are $4.50 for brand registration, $41.50 for standard brand vetting, $15 to vet the campaign, and $10 a month for a standard campaign. Appointment Reminder and EZ Texting state that registration is included in the plan. SimpleTexting charges $4 once and passes carrier fees through at about a quarter cent per message. Spruce charges $19.50 once to enable outbound texting. Ask every vendor who files the registration and whether the monthly campaign fee is in the price, because a rejected or unregistered campaign means your reminders get filtered by the carriers.

What is the best appointment reminder software for medical offices?
For a practice that needs reminders and nothing else, a dedicated tool with a BAA, voice and two-way confirmation is the best fit: Appointment Reminder and Apptoto are the two that publish all of that. If your EHR includes reminders (SimplePractice, Jane, Healthie, IntakeQ), use those first. Large multi-location groups usually end up on a quoted engagement suite.
Can I send appointment reminders from my EHR instead?
Usually yes, and it is often the cheapest route. SimplePractice, Jane, Healthie and TheraNest Premier include reminders at no per-message cost, while TherapyNotes charges 14 cents per text or call and DrChrono charges 5 cents a text past its plan allowance. A separate service earns its place only when you need voice calls, deeper confirmation workflows or multi-location reporting your EHR lacks.
What to ask before you sign

- What happens past the monthly allotment: an overage rate, an automatic upgrade, or reminders that stop?
- Will you sign a BAA on the plan we are buying, and is that plan's price published?
- Who files 10DLC registration, and is the monthly campaign fee included?
- Does it read and write to our EHR schedule, or only to Google and Outlook calendars?
- Can patients confirm, cancel or reschedule by replying, and does that update the schedule automatically?
- Is there a setup fee, an annual contract, or a minimum number of users or calendars?
